Prescriptions must be requested in writing, using your repeat slip, so please keep this safe for future use.
Please leave your request for a repeat prescription at reception or post it to the surgery.
Tick the items you need on the right-hand section of your prescription form. Your prescription will be ready for collection 48 hours later. If you prefer, we can post it back to you (you will need to provide a stamped, addressed envelope).
You can also order your prescriptions online. To register for an account come into reception with proof of ID and you will be given a printout for a login and password.
We are keen to ensure that patients with ongoing medical problems are monitored regularly. If the date for your next review with a doctor or nurse has passed you will be asked to make an appointment.
